Please refer to the EVAA Complaint & Reporting Policy for more information on complaint types and processes—per that policy, this form is for Tier 3 (abuse and violence) and Tier 2 (other misconduct) reports. All other Tier 1 general complaints should be directly routed to relevant EVAA personnel per the Policy . By law, actual or suspected abuse must be reported to law enforcement within certain timeframes.
For all matters, EVAA encourages good faith reports; malicious or bad faith reports will not be tolerated. Please provide as much information as possible to help assist EVAA; providing incomplete or vague information could limit or prevent EVAA from effectively investigating or responding. EVAA will keep reporting parties’ names and contact information confidential within any investigation unless otherwise prescribed by law.
